MANAGEMENT OF YOUR PERSONAL INFORMATION
Your personal health information and your health record may be collected, used and disclosed for the following reasons:

• For communicating with other treating medical professionals

• For follow-up/reminder calls

• For discussion with third party insurers

• Accounting/Medicare/Health insurance procedures

• Disease notification as required by law

• For use by all practitioners in this practice when consulting with you and

• For legal disclosure as required by a court of law

If you have any concerns or wish to restrict access to your information please discuss these with your practitioner or receptionist. This practice adheres to National Privacy Principles (www.privacy.gov.au) and has a written policy available for your perusal.

PRACTICE HOURS (By appointment)

Monday to Friday: 9:00am - 6:00pm
Saturday : 8:30am - 12:30pm (alternative weeks) 

APPOINTMENTS

Please call 9720 7970 for an appointment. Every effort will be made to accommodate your preferred time and practitioner. Appointments are booked at half hourly intervals, although longer appointments are needed for Initial Consultations. If you want a double appointment this can be arranged if needed.

A complimentary SMS reminder will be sent to those who provide us with a mobile phone number. This message will be sent the day before your appointment. We require a minimum of 12 hours notice when cancelling appointments. Late cancellations or failure to arrive for scheduled appointments will incur a fee. New patients that do not arrive for arranged appointments will also be charged a No Show Fee.

There is an after hours message service “to leave a cancellation message”.

AFTER HOURS ARRANGEMENTS

After hours  by calling the practice number 9720 7970. Your call will be put through to our answering machine where you can leave a message and reception staff will call you back as soon as they are available.

TELEPHONE ACCESS

Practitioners in the practice may be contacted during normal practice hours. If the practitioner is with a patient a message will be taken and you will be advised by the reception staff when it is likely that the practitioner will return your call.

SERVICES AVAILABLE

MacFarlane Clinic offers you excellence in manual therapy. A variety of treatment options are available including Osteopathy, Physiotherapy, Chiropractic, Massage and Dry Needling.

Products such as braces, pillows, belts, supports, orthotics and exercise aids are available for purchase or can be ordered to fit your individual needs.

FEES AND BILLING ARRANGEMENTS

Fees are payable at the time of consultation - cash, cheque, EFTpos or credit card payments are acceptable. Health fund rebates can be directly claimed through our Hicaps system on the day. If you have any difficulty paying our fees arrangements can be made through our accounts manager.

Pensioner and health care card holders will receive a discount upon presentation of relevant cards. If eligible, patients can also claim up to 5 treatments on Medicare (this should be arranged through your GP). We also offer a reward program.

THIRD PARTY BILLING

MacFarlane Clinic offers treatment under Work Cover, TAC and Veteran’s Affairs. Direct billing is offered for established claims. Please note, until claims are established, accounts are the responsibility of the patient.

Please be aware there is a small gap fee for workcover and Care plans
